چکیده

Introduction The conservative orthopaedic treatment of hyperkyphosis is less classical than the scoliosis one, because Hyperkyphosis does not affect the respiratory function, and it is more difficult to define normality. The consequences of hyperkyphosis are mainly pain and aesthetic. In English literature very few homogeneous series of results are published. Lowe [1] in a review of the current literature using evidence-based medicine, concludes that when recognized early in adolescence with progressive kyphosis, bracing treatment will usually result in modest correction of the deformity. Sachs [2] analyzing the results of 274 patients concludes that the Milwaukee brace is usually an effective method of treatment for patients who have Scheuermann kyphosis; however, four of fourteen patients who had an initial kyphosis of more than 74 degrees required a spinal fusion. Weiss [3] analyzes the use of transverse correction forces instead of distraction forces with a 3 points brace in 56 patients. In Lyon, we use since 60 years, an historical protocol consisting of: plaster cast reduction, plexidur 5 points brace and specific exercises. Currently, this treatment is performed in day hospital.

Prospective study of 393 adolescent thoracic hyperkyphosis patients treated by the Lyon method

Results Conservative treatment was indicated in 393 patients. 27% of patients do not accept the proposed treatment or interrupt it spontaneously. 23% of patients have a physiological angulation less than 44° at the end of treatment. 43% of patients were reviewed two years after removal of the brace. The initial kyphosis angle was 60.5°. The final angulation 2 years after removal of the brace wa...

Comparing In-Brace Correction, Compliance and Quality of Life in Patients with Thoracic Hyperkyphosis Wearing Milwaukee and Lyon Braces

Background: A thoracic curve degree of greater than 45° leads to thoracic hyperkyphosis, and its common treatment method is orthotic treatment. The acceptance rate of spinal orthosis depends on in-brace correction, patient`s compliance and quality of life. Objective: In this study, we aimed to compare in-brace correction, compliance and quality of life in patients with thoracic hyperkyphosis ...

Successful brace treatment of Scheuermann's kyphosis with different angles

BACKGROUND Scheuermann's disease is regarded as the most common cause of structural hyperkyphosis within adolescents without any proper treatment. It may lead to progressive deformity and back pain which influences patient's quality of life during adolescence and adulthood. Treatment approach to Scheuermann's kyphosis has not been clearly defined due to its different definitions and obscure nat...

Brace treatment for patients with Scheuermann's disease - a review of the literature and first experiences with a new brace design

BACKGROUND In contemporary literature few have written in detail on the in-brace correction effects of braces used for the treatment of hyperkyphosis. Bradford et al. found their attempts effective, treating Scheuermann's kyphosis with Milwaukee braces, but their report did not specifically focus on in-brace corrections. White and Panjabi's research attempted to correct a curvature of > 50 degr...

Curve progression 25 years after bracing for adolescent idiopathic scoliosis: long term comparative results between two matched groups of 18 versus 23 hours daily bracing

BACKGROUND Scoliotic curves do not necessarily stop progressing at skeletal maturity. The factors that influence curve behavior following bracing are not fully determined. Our objectives were to evaluate the loss of the scoliotic curve correction in a cohort of patients treated with bracing during adolescence and to compare the outcomes of 18 versus 23 h of bracing at a mean of 25 years post br...
